Office Hours

Monday through Friday 8:30am to 4:30pm and closed for lunch from 12:30pm to 1:30pm.

 

Please arrive 10-15 minutes early for your appointment to fill out a registration form.

When you come to your appointment please remember to obtain and bring the following:

• Referral letter from Family Physician and or another doctors.
• Bring a valid form of ID such as Driver's License, Passport, and or Military ID.

To cancel an Appointment

Call our office at (469) 854-6809 during office business hours and allow at least 1 day notice so that we can offer your appointment time to patients on our waiting list. If we cannot answer the phone, please leave a detail voicemail with your first and last name as well as date of birth. All of our voicemails are confidential.
